When you complete this form, you will generate a completely formatted Club Charter, which you will just need to print out and sign!

The signed application must be submitted a minimum of 14 calendar days prior to the next scheduled Board meeting for inclusion on the Board meeting agenda.

Submission of this form is required semi-annually by January 15 and July 15.
